Implementation science considers the nuances of target populations, so context is vital to help practitioners determine which framework they should leverage for a specific intervention:

  • Exploration, Preparation, Implementation, and Sustainability (EPIS) Framework:
    • Exploration: This phase involves raising awareness for an issue, which may occur at a federal, state, or local level.
    • Preparation: In this phase, practitioners gather and analyze evidence to determine proper implementation strategies.
    • Implementation: Rolling out the intervention itself depends on several factors, including funding, leadership, and inter-organizational networks.
    • Sustainability: Maintaining an intervention across systems depends on leadership, policy, funding, partnerships, and whether there are other, possibly competing, evidence-based interventions.
  • Consolidated Framework for Implementation Research (CFIR): This model assesses five domains that can influence an implementation’s success:
    • Intervention Characteristics: This may include both essential core components and adaptable components that can be modified to better fit an organization.
    • Outer Setting: External factors, such as economic, political, and social influences, may influence the actual intervention.
    • Inner Setting: Internal factors, such as workflows and organizational culture, can also influence the intervention’s success.
    • Characteristics: This refers to the attributes of the individuals involved.
    • Process: This describes how the implementation is carried out.
  • Promoting Action on Research Implementation in Health Services (PARIHS): This model suggests that the success of an implementation depends on three key elements:

Real-World Application of Implementation Science Frameworks

Implementation science frameworks have helped improve many public health interventions, such as HIV prevention. In this qualitative study, researchers used the CFIR framework to describe the early rollout of long-acting injectable (LAI) pre-exposure prophylaxis (PrEP) in a family medicine setting. They analyzed barriers across the five domains and identified potential solutions to increase LAI PrEP uptake among patients. For example, many providers were not confident in prescribing PrEP (inner setting), so researchers identified a need for training to educate providers about all PrEP modalities to normalize patient-provider discussions about HIV risk.
